Cayuse Submission Forms
The Office of Research Compliance has migrated IACUC protocol management to Cayuse for review and record keeping. If you have any questions about submitting a protocol in Cayuse, please refer to the guide below or reach out to compliance@mtsu.edu. Applications are submitted via the Cayuse IRB portal.

- To request for a new user to be added to Cayuse, fill out the Cayuse New User Request Form (Dynamic Form) below.
The CITI training must be completed prior to seeking IACUC approval. Please refer to the following outline to make your determination.
- Using animals for teaching – Investigators, Staff and Students – Basic Course
- Using animals for testing or research – In addition to the basic course, other courses MUST be completed
- IACUC member – Specific courses need to be completed

Adverse Event Forms
Mandatory reporting of an adverse event is required by PHS Policy and USDA Animal Welfare Regulation.
PHS Policy (IV.C.5) and USDA Animal Welfare Regulations (9 CFR 2.3 (d) (5)) require continued review of previously approved projects.
The IACUC, as part of post-approval monitoring, requires investigators to promptly submit an adverse event form for any unexpected injuries to animals during the course of the project.
Definition: An adverse event is defined as a life-threatening situation to the animal.

NOTE: DO NOT reuse previously completed forms. In an effort to enhance regulatory oversight, the Office of Research Compliance constantly upgrades and revises the forms and notifications to meet Federal standards.
